Fremont Hospital's Outpatient Services Department is located in central Fremont, on the Fremont Hospital campus. We offer a wide range of outpatient mental health services. Our Adult Partial Hospitalization Program (PHP) runs from 9:00 a.m. to 3:00 p.m. Monday through Friday. Our Adult Psychiatric Intensive Outpatient Program (Psych-IOP) runs Monday through Friday, with morning or afternoon treatment options. Chemical Dependency intensive outpatient programming is offered to adults week nights from 6:00 p.m. to 9:00 p.m.
We also offer an Adolescent Partial Hospitalization Program Monday through Friday from 10:00 a.m. to 4:00 p.m. and After-School Program for those adolescents ready to return to a regular school schedule. Although many of our patients drive themselves to treatment, we also provide transportation to and from treatment, as needed. Outpatient Services at Fremont Hospital provides treatment to individuals who live in neighboring communities, including Newark, Union City, Hayward, Castro Valley, Dublin, Pleasanton, San Ramon, Livermore, Santa Clara, San Jose, Mountain View, Campbell, Milpitas, Palo Alto, Redwood City and San Mateo. We are a multidisciplinary team of mental health professionals, including psychologists, social workers, psychiatrists and registered nurses. We specialize in treating individuals who are suffering from moderate to severe emotional distress in one or more areas of their lives. Our team has helped many people who were feeling overwhelmed by their relationships, finances, work or other problems in living. Often, chronic stress exacerbates pre-existing mental illness and contributes to increased anxiety, hopelessness, and depression. Some individuals seeking treatment tell us that they are not functioning as well as they have in the past. Others have said that they are barely able to cope with the demands of daily life.
Upon beginning treatment with us, you will be assigned a primary therapist, a treatment team and a psychiatrist who prescribes medication, as needed. We are qualified to treat a wide range of problems and diagnoses, including depression, anxiety disorders - including generalized anxiety disorder (GAD), post-traumatic stress disorder (PTSD) and obsessive-compulsive disorder (OCD), bipolar illness, borderline personality disorder, dissociative disorders, psychoses and co-existing chemical dependency. Modalities include cognitive behavior therapy (CBT), including skills training based drawn from dialectical behavior therapy (DBT) , mentalizing training for borderline personality disorder and mindfulness-based cognitive therapy (MBCT). Depending upon the needs and situation of each individual, we can help in identifying "triggers" to painful emotions and thoughts - through the provision of an individualized and realistic treatment plan - as well as the essential support so critical in successfully replacing these responses with constructive coping skills and different patterns of behavior. Individuals who acquire these new skills will have the tools by which to positively change the quality and the direction of their lives.
